Acknowledgment of Risk and Assumption of Responsibility

By participating in any activity at DOT Playspace, I, the undersigned parent/guardian and/or participant, acknowledge and agree that participation in play activities involves inherent risks. These risks include, but are not limited to, physical injury, emotional distress, property damage, and, in extreme cases, permanent disability or death. I understand that these risks cannot be completely eliminated despite the best efforts of DOT Playspace staff and management.

Release of Liability and Waiver of Claims

In consideration of being allowed to enter and use the facilities and services provided by DOT Playspace, I, on behalf of myself, my spouse/partner, my children, and any other minors or adults included in my booking, hereby voluntarily and knowingly release, waive, and discharge DOT Playspace, its owners, employees, contractors, agents, affiliates, and representatives from any and all claims, demands, causes of action, liabilities, or damages of any kind—whether known or unknown—arising out of or related to any injury, illness, loss, damage, or death that may occur while participating in activities at or organized by DOT Playspace.

This release applies to all claims resulting from ordinary negligence, including but not limited to:

Personal injury or bodily harm

Damage to personal property

Illness, including communicable diseases

Wrongful death

Medical Emergencies

I authorize DOT Playspace staff to provide first aid if necessary and to seek emergency medical treatment on behalf of my child(ren) or myself if I am unable to do so. I agree to be financially responsible for any resulting medical treatment.

Supervision

I understand that I am responsible for supervising my child(ren) at all times during open play, classes, or any other events at DOT Playspace, unless specifically stated otherwise for a drop-off program. I acknowledge that DOT Playspace is an unsupervised play environment during open play hours.

Agreement and Understanding

By checking “I Agree” below, I confirm that:

-I have read, understand, and voluntarily agree to the terms of this Liability Waiver and Release of Claims.

-This waiver applies to myself, my child(ren), and any additional adults or minors included in my booking.

-I am the parent or legal guardian of the child(ren) listed below and have full authority to enter into this agreement on their behalf.

-This waiver remains in effect for one (1) year from the date of signature and applies to all visits and event participation at DOT Playspace during that period. Waivers must be renewed annually in accordance with DOT Playspace policy.
